Fieldwicks Crushing and Screening Pty Ltd is a privately owned Tasmanian company, which specialises in providing mobile crushing, screening and shredding services. The company has been operating since 1980 from its office and depot at 6174Frankford Rd, East Devonport.

Our services are commonly required in Quarrying, Mining, Construction and Recycling industries. We trust that you will find your employment with Fieldwicks a rewarding experience which provides opportunities to expand and improve your personal career.

Our safety policies and procedures have been established to protect you, your co workers, subcontractors, and visitors while on a Fieldwicks work site. It is important that you understand the potential risks and hazards while working with us, and follow the procedures we have put in place to manage these risks and hazards.

SAFETY POLICY

Fieldwicks Crushing and Screening Pty Ltd are committed to providing a healthy and safe workplace aimed at the elimination of work related injury and illness. Management will take all possible measures to reduce the risk to the health, safety, and welfare of all its employees, authorised visitors, and contractors. We are committed to complying with all relevant legislation, including regulations, standards and codes of practice.

RESPONSIBILITIES

Management will

  • Provide a safe and healthy working environment
  • Provide safe systems of work
  • Provide training and supervision that can be reasonably expected to assist employees to work safely.

Application of Policy

This policy requires the cooperation of all management, employees, and visitors. We encourage suggestions to allow us to continually improve the health and safety in our workplace. This policy applies to all business operations on all sites.

Fieldwicks Crushing and Screening Pty Ltd want to encourage a safe and pleasant work atmosphere. This can only happen when everyone cooperates and commits to appropriate standards of behavior.

The following is a list of behaviors that the company considers unacceptable. Any employee found engaging in these behaviors will be subject to disciplinary actions including reprimand, warning, layoff, or dismissal:

  • Failure to be at the work place, ready to work, at the regular starting time.
  • Fighting or engaging in horseplay or disorderly conduct.
  • Refusing or failing to carry out any reasonable instructions of a supervisor.
  • Intentionally giving any false or misleading information to obtain employment or a leave of absence.
  • Sexual harassment
  • Using threatening or abusive language toward a fellow employee (bullying).
  • Falsifying timesheets or any records.
  • Smoking contrary to established policy or violating any other fire protection regulation.
  • Failing to wear clothing conforming to standards set by the company.
  • Being tardy or taking unexcused absences from work.
  • Not taking proper care of, neglecting, or abusing company equipment and tools.
  • Using company equipment in an unauthorized manner.
  • Possessing firearms or weapons of any kind on company property.

Instant Dismissal may apply if one of the following occurs:

  • Willfully damaging, destroying, or stealing property or equipment belonging to fellow employees or the company.
  • Coming to work under the influence of alcohol or any drug, or bringing alcoholic beverages or drugs onto company property (3 strikes rule).
  • Willfully violating safety or health regulations.

Fieldwicks have a zero tolerance policy regarding drugs and alcohol in the workplace. Any person working under the influence of drugs or alcohol is putting themselves and others at risk.

Please notify us if you are taking medication that may affect your behaviour at work. This information will be kept strictly confidential.

Fieldwicks Crushing and Screening Pty Ltd reserve the right to carry out random drug and alcohol checks on any employee at any time.

Fieldwicks Crushing is committed to providing a healthy work environment for all employees. Fieldwicks acknowledges the health risks associated with smoking (including e-cigarettes) and passive smoking/aerosol.  Fieldwicks have an obligation to protect employees and visitors from airborne contaminates relating to these products.

The organisation will take all reasonably practical steps to provide a healthy work environment to workers and visitors by complying with the below objectives in place to minimise the risks.

  • Smokers are to smoke in designated areas or obtain a reasonable distance from other personnel when smoking to avoid second hand smoke to non-smokers.
  • Prohibit smoking in company vehicles and equipment.

Fieldwicks isolation procedure is used to prevent a machine being operated while under repair or maintenance. A personal danger lock and tag must be used to isolate a machine prior to any maintenance or repairs being carried out.

Locks and tags can only be removed by the person who placed them on the machine or an authorised person approved by management.

Manual Handling refers to any activity requiring a person to lift, lower, carry, push, pull, or restrain any object and is the most common cause of injury in the work place.

It is Fieldwicks policy to provide all employees with a safe and healthy workplace by identifying, assessing and controlling manual handling risks.

While management is responsible for the health, safety and welfare of all staff, all employees must report potential and actual manual handling hazards.

Correct handling techniques must be used at all times. Never lift or manually handle items larger or heavier than you can easily support. Think about what you need to move, how heavy it is, how far it is to be moved, whether twisting or bending is involved and whether mechanical aids are available. If you are in any doubt - ask for help.

Working extended hours, performing repetitive tasks, varying temperatures, inadequate rest breaks and lack of good nutrition is a major cause of fatigue in a workplace. What is fatigue? Fatigue is the body’s response to continued physical, or mental activity, or sleep loss.

Signs of fatigue include:

  • Difficulty keeping eyes in focus
  • Poor decision making
  • Slow response time
  • Drowsiness
  • Bad temper

 

Workers are required to have a minimum of 10 hours break from the time that they arrive home from work, to when they leave for work the following day.

When suffering from the effects of fatigue, they are safety hazard to both themselves and others. If they are fatigued, they will cease work immediately.

Symptoms of Operator or Driver Fatigue

  • Drifting
  • Intermittent braking
  • Random variations in vehicle speed
  • Following too closely behind another vehicle
  • Poor control or inaccurate use of operating equipment

 

What You Can Do

  • Pay attention to the signs of fatigue your body is giving.
  • Make getting enough continuous sleep an important priority.
  • Take a 30-minute nap when you are feeling fatigued.
  • After sleep, make sure you are over sleep inertia before you start driving (about 15 minutes).
  • Use caffeine only when you are fatigued, and allow at least 15 minutes for it to take effect.
  • Don't drink caffeine 3-4 hours before you want to go to sleep.
  • Take breaks and exercise after every 2-hour period of operating machinery or driving.

To prevent serious falls in the workplace all fall hazards (greater than 2m) need to be identified, risk assessed, and controlled. Whenever possible, working at heights should be eliminated. When working on crushers use the handrailed walkways, or stay within the feeder bin. Do not walk around on top of the machinery. We can also commonly use gravel pads to work off or utilise the handrailed zones on our excavators and loaders. If this is not possible a hand railed platform, ladder, or fall arrest system may be required.

Always use three points of contact when climbing on and off machinery.

Fieldwicks are committed to providing a safe working environment for its personnel and preventing injuries due to falls. 

Managing the Risk of Falls 

  • Identify any reasonably foreseeable hazards that could create a genuine fall risk
  • Eliminate the risk so far as is reasonably practical
  • If it Is not reasonably practical to eliminate the risk, then minimise the risk by implementing control measures.
  • Consult with workers to identify and control risks
  • Control measures must be maintained so they are effective.
  • Review control measures to maintain effectiveness. 

Control Measures 

If a worker is working above 2 meters in height or if there is a reasonable risk of falling that would result in an injury, a risk assessment must be completed and controls put in place to minimise the risk as far as practical. For regular tasks a safe work procedure can be developed as an alternative to the risk assessment process.

A worker is not considered to be working at heights if above 2 metres in height but working within a fixed walkway or platform with handrails. 

Typical control measures are : 

  • Use earth mounds to eliminate risk
  • Use handrailed sections of plant as work platform (must be shut down and locked out)
  • Platform ladders
  • Install temporary guard rails
  • Fall arrestors
  • Elevated Work Platforms

Long term dust exposure can have long term affects on your health and wellbeing. Primarily the risk is through inhalation of dust.

To minimise exposure, where possible we use dust suppression systems in quarry operations. In addition, while operating crushing plant, machinery doors and windows should be kept shut. Cabs and cab filters should be kept clean. Dust masks/respirators are always available for use when conditions require.

In providing a safe and healthy workplace with the aim of eliminating injury and illness, Fieldwicks have developed a series of Safe Work Procedures for the correct operation of machinery and the carrying out of tasks. These guides to safer work techniques will not tell you all you need to know about operating the item of plant, but they will endeavour to point out some of the common hazards and how to avoid them. Only experience will teach you some things, but following guidelines and talking to experienced operators will help you to work safe.

This list in not exhaustive as these procedures will be continually updated as and when the need arises.

All employees will be required to read and understand these procedures before the operation of plant and machinery and are asked to seek clarification if any doubt remains as to the correct use of equipment. 

A hazard is a thing or a situation with the potential to cause injury or illness to a person. All personnel have obligations to identify hazards in their work sites and this is the first step in minimising accidents and incidents.

Before commencing work on a site, a worksite assessment must be undertaken. This includes the completion of a Job Safety Analysis, with the aim of clearly identifying and controlling hazards in the workplace. An assessment of the risks needs to be undertaken so that control measures can be determined and implemented for that particular site.

Controlling the risk of exposure to workplace hazards can be accomplished by a number of methods including elimination of risk, substitution and isolation.

FC&S recognises its safety duties under the Heavy Vehicle National Law to ensure the safety of its transport activities.

FC&S is committed to the provision of service and maintenance of its heavy vehicle assets to ensure the safety of all within our operations.

In implementing this policy FC&S will:

·         Continue a consultative approach with our staff, contractors, suppliers and clients.

·         Apply risk management principles in the identification, assessment and control of risks associated with all of our heavy vehicle activities.

·         Provide adequate information and supervision in Chain of Responsibility responsibilities.

 

 

Compliance with Chain of Responsibility legislative requirements is the responsibility of all FC&S staff and contractors, to ensure the safety of the general public, staff and heavy vehicle drivers.

Under the Workplace Health and Safety Act 2012 an employee is required to comply with any safety direction given by the employer and this includes the wearing and care of Personal Protective Equipment.

Where PPE and clothing has been supplied for a variety of work procedures and conditions, employees are reminded that the following rules apply:

  • No equipment, no start
  • At all times, the equipment is to be used for its designed purpose
  • After any use, the equipment is to be cleaned and stored in an appropriate manner
  • All issued equipment will comply with the Standards Association of Australia

As PPE requirements will vary from site to site depending on regulations and hazards at each site, reference should be made to the particular Site Safety Information for information on the requirements at each location.

Unless otherwise directed, the following minimum requirements are to be met:

  • Safety Boots - To be worn on all sites and workshop areas
  • Safety Vests - To be worn on all sites
  • Safety Helmets - Must be worn if working near operating plant
  • Safety Glasses / Goggles - Must be worn if using cutting or grinding equipment of any kind, or if close to cutting and grinding equipment in use
  • Sunscreen / Sun Hat / Sun Glasses - To be worn when working outdoors
  • Ear Plugs / Muffs - To be worn when using or exposed to equipment or machinery that emits excessive noise.

All employees will be issued with the required safety equipment when needed. Employees who lose or damage equipment through neglect may be required to replace the equipment at their own cost. Equipment will be replaced on a fair wear and tear basis. 

First Aid Kits are supplied in all company light vehicles. It is the individual employees responsibility to familiarise themselves with the contents and basic instructions supplied with these kits. Employees are also responsible to check the contents of these kits and advise management of any replacements required. Management will review first aid training needs and the inspection, location and quantity of first aid kits on an annual basis.

All injuries and near misses must be reported to the office and documented on an accident / incident form or timesheet.

A fire extinguisher is provided with every company light vehicle and items of large plant. It is the employees’ responsibility to advise their supervisor of any defects in these items. Accredited fire services technicians regularly check the compliance and condition of these extinguishers.

Should a fire be detected at a work site, immediately alert at least one other person, but avoid panic. Attempt to extinguish the fire only if trained and it is safe to do so using available fire fighting equipment. If the fire cannot be easily contained immediately, raise the alarm and contact Emergency Services.

An emergency is a situation that poses an immediate risk to health, life, property, or the environment that requires assistance from professional emergency services.

An emergency exit location map is displayed at our office complex and all exits are marked clearly. It is the employees responsibility to familiarise themselves with the exit nearest their work station and the Emergency Assembly Point.

The Emergency Assembly Point for an external work site will be clearly identified on the Job Safety Analysis.

In an emergency situation, it is important to “stay calm” and follow all instructions given by professional emergency services.

In the event of a hazard, incident, accident or near miss on the work site, verbal notification must be given to your supervisor as soon as possible. It there is an immediate risk of injury, personnel should take such action as to make the area safe, ensuring their own safety and that of others on the site.

An accident / incident report form needs to be filled out by the individual involved within 24 hours of the occurrence. Upon receipt of this form, an investigation will be undertaken to determine the cause of the incident and any corrective action required.

Minor incidents / near misses can be documented on your timesheet but please still ensure that the office or your supervisor is notified.

Where employees are issued with uniforms, you will be expected to wear the uniform and keep the uniform in a clean and presentable condition.

Employees are expected to maintain their hair in such a manner as to reduce the possibility of injury. The same requirement applies to jewellery, belts and other items of apparel.

During your term of employment with us, if you are required to be fitted with a pacemaker please let us know. Many of our machines are fitted with large magnets which can affect pacemaker operations and be extremely dangerous.

Fieldwick Crushing and Screening Pty Ltd are committed to conducting business with consideration to environmental and social impacts associated within our industry. This policy has been developed to ensure the deliverance of services in an environmentally responsible manner.

We are committed to complying with all relevant legislation, including regulations, standards and codes of practice.

The following 'Environmental Objectives’ have been established as the minimum standard required for all managers, employees, subcontractors and suppliers. These objectives shall be achieved through the development and implementation of a system based on the ISO14001 standard.

Environmental Objectives:

  • Ensure that all aspects of contractual, statutory and regulatory environmental requirements and obligations are addressed.
  • Commit to constant improvement with regard to environmental performance, reporting and procedures.
  • Commit to the prevention of unessential pollution
  • Apply reasonable practices to reduce the potential of any adverse environmental impacts
  • Ensure that adequate resources are available to operate in an environmentally responsible manner
  • Train employees to promote awareness of environmental issues
  • Be respectful and considerate to local environment and community in which we operate
  • Encourage practices that reduce waste including preventative maintenance of machinery, energy conservation, and recycling of material where possible.
  • Treat environmental issues equal with economic, quality and safety issues.
  • Set specific objectives which will be established and managed through a corrective and preventative action process.

Trial Period

All new permanent employment positions are offered conditional on the successful completion of an initial three month trial period. During this time you will be trained in our procedures and be assessed as to your compatibility for the position. This trial period should also be used to make your own assessment as to whether this position is suitable for you on a long term basis.


Pay Arrangements

Pays are processed weekly with the pay week running from Wednesday to Tuesday. Your pay will be directly credited to your bank account and will usually be available on a Thursday morning. To facilitate the processing of your wages, you must accurately complete your timesheets daily and forward them to the office. We appreciate that your employment may take you away from our Head Office location and request that these timesheets are returned at least once a week. Failure to submit timesheets may result in a delay to the processing of your wages.

If you feel that an error has been made in the calculation of your pay, please notify the office before the next pay period ends.


Workers Compensation

We are committed to providing you with a safe working environment, however accidents do happen. If you suffer an injury or illness that you believe was caused by your employment, it must be reported to the office immediately and an Accident Incident Report Form completed.

As a result of this incident, if you wish to make a claim for compensation for the injury under the Workers Rehabilitation and Compensation Act 1988 you should do so as soon as practicable. This claim must be made on the appropriate form and a copy of the form is available from the office. A Workers Compensation Medical Certificate that has been issued by an accredited medical practitioner must be attached to this claim.

Assistance in relation to the injury or claim for compensation is available from the office.


Hours of Work

An employees employment is based on a 38 hr week. However provided there is work available we expect employees to work on average 10 - 12 hours per day, 5 days a week. Some weekend work will also be required from time to time. These hours are to be worked between 6:00 am and 6:00 pm, or as agreed between the employer and employee.

Employees are required to take an unpaid half hour meal break within five hours of employment for the day commencing along with a paid rest break of ten minutes per day.

The time of taking scheduled breaks may be altered by agreement between employees and employers, to maintain the continuity of operations.

All hours worked in excess of the standard 38 hours per week will attract overtime rates of pay.


Annual Leave

Annual leave is provided for in the National Employment Standards and for each year of service with Fieldwicks Crushing and Screening Pty Ltd you will be entitled to four weeks (152 hours) of paid annual leave. This leave accrues progressively during the year of service and accumulates from year to year.

Paid annual leave may only be taken for a period agreed to by the employee and Fieldwicks Crushing and Screening Pty Ltd.

Annual leave cannot be cashed out if the cashing out results in the employees remaining accrued entitlement being less than four weeks (152 hours).


Personal leave

Personal leave is provided for in the National Employment Standards and for each year of service with Fieldwicks Crushing and Screening Pty Ltd you will be entitled to ten days (76 hours) of paid personal leave. This leave accrues progressively during the year of service and accumulates from year to year.

Personal leave may be taken because the employee is not fit for work due to a personal illness or injury, or to provide care and support to a member of the employee’s immediate family.

If required, evidence must be provided to Fieldwicks Crushing and Screening Pty Ltd to support the taking of personal leave.


Working away

Due to the nature of Fieldwicks operations, periods of working away from our home office location are common. When travelling to another location, ordinary time pay rates will apply to all time in excess of that usually spent in travelling to our home office location.

When required to stay away overnight, Fieldwicks will provide full accommodation and meals for the duration of the stay.

An employee will be supplied with a mobile phone if it is deemed necessary to their position. This phone is provided to facilitate communication between the employee, management and emergency personnel.

Private usage of the mobile phone should be kept to a minimum. If it is found that an employee is using their phone irresponsibly, that employee may have the phone removed and be requested to reimburse Fieldwicks Crushing and Screening Pty Ltd for excessive personal call or usage charges.

The use of a mobile phone while driving is illegal.

All employees who have been allocated a mobile phone are required to take the utmost care and responsibility for the phone. If a phone is lost or stolen, it should be reported to your supervisor immediately. Fieldwicks will be responsible for replacing the phone, however in circumstances where it has been shown that the employees carelessness contributed to the loss of the phone, the employee may be required to contribute to the replacement cost of the phone.

I acknowledge that I have been given a copy of the Fieldwicks Crushing and Screening Pty Ltd Induction Booklet and have read and understood the information contained within.

I have watched and listened to the Access and Egress Video and understand the requirements for climbing on and off machinery.

I agree that as an employee of Fieldwicks Crushing and Screening Pty Ltd, I have a responsibility to:

  • Take reasonable care for my own safety and the safety of any persons working around me
  • Comply with all relevant legislation, statutory rules, codes of practice and standards
  • Comply with all aspects of Fieldwicks Crushing and Screening Pty Ltd Workplace Health and Safety Policy and associated management system
  • Report all near misses, incidents, hazards and accidents to Fieldwicks Management immediately and assist in the compilation of Incident Report Forms and the control of reported hazards.
  • Ensure Fieldwicks Crushing and Screening Pty Ltd instructions relating to health and safety are carried out at all times
  • Ensure all my Personal Protective Equipment is in good condition at all times
  • Participate in relevant training programs as requested
